Request for Transcript

Please follow the following steps to request a transcript:

1. Please click here to download the request for the transcript form.
2. Print and completely fill out the form, with your signature and date.
3. Scan or take a picture of the signed transcript request.
4. Send an email to [email protected] and attach the form. Do not hand over the form to anyone.
5. Your transcript will be mailed to you. For any questions, please send an email to [email protected].
6. You are required to pay by credit or debit card at pitc.edu and attach the payment to the transcript request email.
